Booking Agreement

This agreement lays out the expectations from ‘The Diabetes Lady’ and you/yourselves on your Diabetes Health and Wellbeing journey.

This is about you, the person living with or without diabetes and those who love and care about you.

Honesty and transparency is required from you the person living with diabetes at all times, or those requiring phlebotomy services, as anything left out may cause you issues. The Diabetes Lady cannot be held responsible for adverse outcomes due to information/health issues that have not been declared.

For those with a diagnosis of diabetes, it is a progressive disorder, and therefore The Diabetes Lady cannot be held accountable for any complications arising from the lack of/or misinformation provided by you or others.

On making an inquiry and booking an appointed time slot, the agreement starts and so too does the expectations from both parties.

N.B: In no way does this replace your sessions with your GP/Family Doctor/ Diabetes Team – it is additional and here as educational and support. Any suggested changes to medication or treatments will need authorizing by your GP/Family Doctor or regular Diabetes Team. **So will need to be informed in writing. I therefore I will need this information.

Contact details

There is a need for a name/current email and up to date contact telephone number, initially (all encrypted and secure, in compliance with data protection).

On initial booking there is a Health and Well-being assessment form, like any good practitioner it is important to know and understand your health journey to make appropriate decisions regarding your care. This is all kept encrypted in line with GDPR and Data Protection. I am a member of Information Commissions Office (ICO).

Prices

Your chosen time slot is provisionally booked, the costs are as follows (and are subject to change as the company costs change (as you can understand)

Blood Tests (Phlebotomy)

All appointments are secure face to face (f:f) from £50 for a 15 minute session, for taking of bloods, you order and pay for your tests via the store, to an outside provider. (Forth and Partners)

Diabetes Initial Assessment - Coaching Motivation and Education

Offering face: face or virtual consultations, to look and assess your diabetes journey, whether long-term or newly diagnosed.

These sessions are for up to an hour, and we talk and discuss your priorities and concerns.  As a new client I will also carry out routine checks that may or may not have been carried out before. For this session my prices start at: £150

Diabetes Follow-up Assessment

This is a half-hour session were we carry on from our last session and I can provide more support to help keep you motivated and up date your diabetes education. (Bloods may be needed, additional cost) £80

Full Diabetes Quarter Package (making a difference package)

I am all about you believing in you and taking control of your diabetes - so I thought it might be a good idea in offering a package of care - there are those that like to dip in and dip out but a lot of us like continuity.  Especially with our diabetes care.  So to offer something, especially for those who haven't experienced my style of working, I thought it may help if there is an opportunity to allow you to see what difference you are making on your diabetes journey.

I am happy if you have your bloods done via your GP Practice but I know that Surgeries are overwhelmed and sometimes unable meet the demand. So I can provide this service.

So for this package I will include the **Initial Assessment (60 minutes), 3 x one monthly follow-ups (30 minutes each)  I would repeat bloods at the last session, which will be a total of 4 months - ** At the initial 60 minutes I will do a Full Diabetes blood screen and at the last 30 minute an HbA1c.

At the time of writing, this package should cost £551.  But I am offering it to you at £390, giving you a saving of £261.

Payment must be made at the time of booking via Square (safe and encrypted)

PLEASE NOTE: I can share our consultations with your GP, but only with your consent.

However, if a serious health issue is discovered and I need your GP's input I will forward the information found, to ensure your safe well-being. 

Health records are kept virtually and encrypted and I am registered with ICO (Information Registered Office). For your peace of mind and safekeeping.

Booking

On receipt of booking and payment, you will receive a provisional booking acknowledgement, with this a Health Questionnaire Form, and a more detailed contact form. We ask for your GP (Family Doctor) and about all your health issues. You will need your latest full diabetes blood screen (no longer than 3 months) and not just your HbA1c. This is to ensure that no other health issues in relation to your diabetes are missed. *this is if you have not opted into the phlebotomy service package.

Please fill in and return the Health questionnaire within 48hrs of receipt.

You will still need to attend Annual Diabetes Reviews, Foot Screenings and Retinal Screen with the Retinopathy Team, and GP Practice.

On receipt of forms and payment your booking is confirmed

You will receive a reminder of your booking 2 days before appointment, the day before and 2 hours before your allotted time.

Cancellations and Rebooking

The Diabetes Lady realises that life sometimes gets in the way of what we are wanting to do. As in planned appointments.

If you need to rebook your appointment, I understand, and this can be done.

I would prefer that you do this before your first reminder 2 days before your appointment – giving me time to rebook the slot.

Cancellations

I will deduct 10% admin fee from your original fee if cancelled within 4 weeks prior to your appointment, but, not if you rebook at the time of cancellation. 

A 50% retainer fee will be payable if cancelled less than 7 days before and if you do not rebook at the time of cancellation.  If less than 48 hours or no notice of a rebooking before your appointment date 100% of the fee will be payable and not refunded because I may not be able to rebook the session and the blood test will have been ordered in your name.

Provision of the Service and How to contact ‘us’

The Diabetes Lady trade name for Diabetes Complete Care Ltd provides an on-line cloud-based application on booking an appointment, to enable The Diabetes Lady to better manage and aid you (“the client”). Health and Medical conditions disclosed by you (“the client”) enables The Diabetes Lady to aid your personalised self-management based upon you the individual and your current health status. (“services”)

You acknowledge that you have provided The Diabetes Lady with accurate and complete registration information on completing your booking which shall include any medical bona fides as may be reasonably requested by The Diabetes Lady.
